Aviation Safety X Report:
On January 27, 2025, a Robinson R44 Raven II helicopter, registered as PR-TIB and operated by AeroGreen Aviação Agrícola Ltda, was involved in a fatal crash near Cruzília, in southern Minas Gerais, Brazil. The aircraft, manufactured in 2009 and primarily modified for agricultural purposes such as crop spraying, had three occupants onboard: the pilot, Fernando André Ferreira (46), and two passengers, Lúcio André Duarte (39) and his wife Elaine Moraes Souza (37), both employees of the farm where the helicopter was based.

The incident occurred during a recreational flight undertaken at the pilot’s invitation. After returning from another farm, the helicopter was observed attempting to land at Fazenda Chalé when witnesses reported unusual maneuvers. According to accounts, the aircraft appeared to spin unexpectedly before striking the ground nose-first. The resulting crash caused the helicopter to catch fire, leaving no survivors. Emergency services, including the São Lourenço Fire Department, responded promptly, extinguishing the flames and recovering the victims’ remains.

Preliminary investigations suggest potential mechanical failure or operational issues during the landing phase. The helicopter was reported to have a valid airworthiness certification for private and agricultural use but was not approved for commercial passenger transport. Local employees noted hearing unusual sounds before the crash, indicating the possibility of a technical malfunction. The Brazilian Aeronautical Accident Investigation and Prevention Center (CENIPA) and the Third Regional Service for Aeronautical Accident Investigation (SERIPA III) have been tasked with determining the exact causes of the accident.

The loss of this flight highlights the complexities of aviation safety in general-purpose operations such as agricultural work and private use. Investigators are expected to analyze data from the wreckage, interview witnesses, and assess maintenance logs to uncover contributing factors. The findings will provide valuable insights into improving safety protocols and mitigating risks in similar future operations.
